A simple accident timeline format

  1. Start with the crash event. Record date, time, city/state, ZIP if known, vehicles involved, police/crash report information, photos, witnesses, and a short factual description.
  2. Add injury and treatment milestones. List when symptoms started, ER/urgent-care visits, medical follow-ups, imaging orders, therapy/chiropractic visits, prescriptions, bills, and discharge notes.
